To be included and to belong, we transformed congregate day services by individualizing supports.
The traditional group based, Abilities Centre program transformed into individualized community one-to-one supports through the new Community Connections and Passport Services.
People choose what they want to do, we connect them with Community Connections team members and we provide the supports for people in their community. Hockey games, art classes, swimming, sailing . . . we support it all.
